Add a configurable form to the available page slices. This is an extension on the base slice supplied by the Silverstripe Pageslices package.

Unfortunately UserForms requires to be a page so in order to get this slice to work the slice creates a hidden UserDefinedForm page and links it to the slice. Any edit you do on the form on that form will be picked up by the slice. When the slice is deleted the created UserDefinedForm page is cleaned up as well.
